BEHAVIOR CONTRACT

In order to help my classmates and myself learn, achieve and enjoy my class as much as possible this year, I agree to follow each of these rules:
1. I will respect others people’s dignity and property.
2. I will have a positive attitude. I will discuss legitimate concerns reasonably and respectfully.
3. I will demonstrate good sportsmanship at all times.
4. I will not disturb others while Mr. de Santos is teaching or my classmates are working.
5. I will be prepared for class. (This includes having homework, books, supplies, etc.)
6. I will follow all district and school rules and procedures.
7. I will give 100% effort every day.
For following these rules, I will be rewarded with praise, positive notes and prizes and most of all, increased knowledge and self-discipline.
If I break a rule, I will face the following consequences:
1st time: Verbal Warning
2nd time: Mr. de Santos will note my behavior in my daily planner for my parents to read and initial.
3rd time: same as 2nd time consequence and a call to parents.
Severe disruption: Parent conference scheduled immediately.
In a Nine Weeks Period, if I break the rules three times on three different dates (with parent notification each time), I will earn a Needs Improvement on my report card for that period.
